Dear Parent/Guardian,

The Hawaiʻi State Department of Education (HIDOE) is writing to inform you of a recent data security incident involving Hazel Health, a contracted provider of virtual mental health services for HIDOE students.
Hazel Health recently notified HIDOE that an unauthorized third party gained access to a portion of its network environment. Upon discovering the incident, Hazel Health initiated its incident response process and engaged independent digital forensics specialists to investigate.

At this time, Hazel Health is still determining whether any HIDOE student or Department information was accessed and, if so, what information may have been involved. The investigation is ongoing.

As a precaution, HIDOE has temporarily paused new Hazel Health services while we work with Hazel Health to better understand the scope of the incident and determine whether any HIDOE-related information was affected. Students who are currently receiving services through Hazel Health may continue their current course of services while the investigation is underway. Please be aware, however, of the ongoing potential risk to students’ protected health information (PHI). Parents and guardians may also choose to discontinue their child’s Hazel Health services.

Please complete this form to indicate whether you would like your child to continue or discontinue their current Hazel Health services.
If you choose to discontinue Hazel Health services, please contact your child’s school. School staff will work with you to identify appropriate next steps to support your child’s mental health and well-being.

HIDOE will provide additional information and guidance as it becomes available, including information about whether HIDOE student data was affected.
We understand that this incident may raise concerns about the privacy and security of your child’s information. Protecting student information is a priority for the Department. We appreciate your patience and understanding as we work with Hazel Health to obtain additional information and determine any necessary next steps.
